29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing:  (a)  On or about 11/19/15 - Production areas, the intermediate rail was missing from the  guardrail system(s) installed around the pit that contained the Form stripping equipment at production line #6.  (b)  On or about 11/19/15 - Production areas, the intermediate rail was missing from the guardrail system(s) installed around the pit that contained the Form stripping equipment at production line #8.  c)  On or about 11/19/15 - Production areas, the intermediate rail was missing from the guardrail system(s) installed around the pit that contained the Form stripping equipment at production line #9.  d) On or about 11/19/15 - Production areas, the intermediate rail was missing from the guardrail system(s) installed around the pit that contained the Form stripping equipment at production line #10.

29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers shall be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section:  (a) On or about 11/19/15 - Potassium Hydroxide tank, the standard rails installed on both sides of the stairs were missing the intermediate rail.  (b) On or about 11/19/15 - Compounding mix tanks, the standard rails installed on both sides of the stairs were missing the intermediate rail.

29 CFR 1910.101(c): Compressed gas cylinders, portable tanks, and cargo tanks did not have pressure relief devices installed and maintained in accordance with Compressed Gas Association Pamphlets S-1.1-1963 and 1965 addenda and S-1.2-1963:   (a)  On or about 11/19/15 - Production Line #5, the pressure relief valve on the Superior Gas Chlorinator was not vented to a safe location.  Employees working in the production area were exposed to release of chlorine if venting occurred at the metering valve.
