29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: At or about November 18, 2015 and at times prior thereto, at the jobsite located at or near 1652 S. Fred Hanna Drive, Fayetteville, Arkansas, the guard on the trim saw was inadequate because it not prevent the operator from having his hand in the danger zone while placing wooden boards into the in feed chute of the saw to cut 6 inch boards. This condition caused the amputation of an employee's finger.
