29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about November 20, 2014 , and at times prior thereto, the employer did not provide machine guards to the following equipment, exposing the employees to the hazards of being caught in rotating parts.   a. In the Press Department on the Gammerler STC-70 Stacker interlocks had been by-passed, permitting access to rotating parts and paper chain assembly.  b. Next to the dock area the Galbreath OPR2290 Trash Compactor interlock had been by-passed, permitting employee to operate it with the safety gate open or closed.   c. In the Mail Room on the K & M HL2000 Inserter interlocks had been by-passed, permitting access to moving parts.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures, or by any of the following means identified in sub-paragraphs (A)through (D) of this paragraph:  On or about November 20, 2015, and at times prior thereto, the access panel on the back side of the Gammerler Stacker was not installed leaving live electrical parts exposed, exposing employees to the hazard of electrical shock and burns.
