OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by flying projectiles:      Mounting Department:  On or about November 18, 2015, employees was exposed to the hazard of being stuck by flying projectiles, while operating a compressed air blow-off gun to clean prints prior to lamination.  The compressed air used by this blow-off gun was transported above grade via 1/2" SCH 40 PVC Charlotte Pipe at an operating pressure of approximately 103.8 p.s.i.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  Mounting Room:  On or about November 18, 2015, employees were exposed to an electrical hazard, while operating a Neolt Trimmer, Model EL PW TRIM 130, with a 120 Vac power cord that had damage to the outer jacket, thus exposing the internal wiring.
