29 CFR 1910.213(m)(1): The cutting heads of each wood shaper, hand-fed panel raiser, or other similar machine not automatically fed, shall be enclosed with a cage or adjustable guard so designed as to keep the operator's hand away from the cutting edge. The diameter of circular shaper guards shall be not less than the greatest diameter of the cutter. In no case shall a warning device of leather or other material attached to the spindle be acceptable:    (a)  On or about November 19, 2015, in the Door Department, employees were not trained on the Lockout/tagout program to understand when equipment needed to be shut down, locked out, and de-energized prior to performing cleaning activities.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (a)  Employees who performed cleaning activities were not required to:  (1)  know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy,  (2)  turn off and shut down the machines  (3)  physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4)  affix lockout or tagout devices to each energy isolating device,  (5)  prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.    On or about November 19, 2015, in the Door Department, employees cleaned the work surface of the Ritter Manufacturing Inc (Model:  R10125753, S/N:  1310) with an air nozzle while it was still energized exposing employees to struck-by hazards.
