Certification records which include the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the crane which was inspected shall be made monthly on critical items in use such as brakes, crane hooks, and ropes. This certification record shall be kept readily available. a. On or about November 23, 2015, the employer did not have inspection records on truck-cranes used to load/unload rubber tires on site and off site. Employees were thereby exposed to struck by hazards from handling rubber tires with truck-cranes.

29 CFR 1910.184(e)(1): Sling identification. Alloy steel chain slings shall have permanently affixed durable identification stating size, grade, rated capacity, and reach. a. On or about November 23, 2015, the employer required employees to use alloy steel chains as part of the load manipulation and steel chains did not have their load capacity identification. Employees were thereby exposed to struck by hazards during load manipulation while using truck-cranes. The conditions found on truck-crane's components were the following: 1. Three (3) out four (4) alloy steel chains were missing their identification tag - load attachments on truck # 50. 2. One (1) alloy steel chain out of two (2) was missing its identification tag - load attachment on truck # 39. 3. On truck-crane # 42 - all alloy steel chains were missing.

29 CFR 1910.184(e)(3)(i): In addition to the inspection required by paragraph (d) of this section, a thorough periodic inspection of alloy steel chain slings in use shall be made on a regular basis, to be determined on the basis of (A) frequency of sling use; (B) severity of service conditions; (C) nature of lifts being made; and (D) experience gained on the service life of slings used in similar circumstances. Such inspections shall in no event be at intervals greater than once every 12 months. a. On or about November 23, 2015, employees were required to use alloy steel chains slings to manipulate loads (tires) during removal/installation and yearly inspections were not performed. Employees were thereby exposed to struck by hazards from manipulating rubber tire loads with truck-cranes.
