29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: (a) Men's Restroom, near Front Entrance - The men's restroom, provided for employee use, had become soiled, including the toilet and sink, exposing employees to unsanitary conditions, on or about December 3, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): (a) Dock Area - An employee operated a Toyota propane powered sit-down style forklift, serial number 5FGC25-14516. The employer did not certify that the employee had received the required training and evaluation, with a certification which included the name of the operator, the date of the training, the date of the evaluation, and the identity of the person performing the training or evaluation, on or about December 2, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
