29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard:  In the shop guards or barriers were not installed on the sides of the power press known as "greaser" to the left side and the right side of the operation that would prevent the operator or passers-by from reaching into the point of operation. Employees were exposed to a struck-by and/or a caught-in hazard.

In the shop certification records of the inspection and maintenance of power presses were not maintained/available. Employees were exposed to a struck-by and/or a caught-in hazard.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  At the business location located at 524 NW 20th Ave, Kingman, KS; the employer has not updated the hazard communication program and trained employees on the portion of the hazard communication program that explains the labels on shipping containers and the safety data sheets for items such as but not limited to fuels, lubricating oils and greases including the order of information and how the employee could obtain and use the hazard information.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  Employee(s) perform servicing and maintenance activities on equipment including, but not limited to, Niagra Full Revolution Punch Press Models A5 and A6 and 1/2 as well as Clearing Knuckle Joint Punch Press Models KS-1250-30 and KS-1400-36 Part Revolution Punch Presses which had multiple energy sources including compressed air and electrical power. The employer did not ensure that the annual/periodic inspection of the energy control procedures were being performed and documented.
