29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) On or about 12/04/15 - in the Shop, point of operation guarding was not provided on the Standard Industrial Corporation Press Brake model AB200-12 when an employee was within reach of the point of operation and using the foot pedal.  (b) On or about 12/04/15 - in the Shop, point of operation guarding was not provided on the Standard Industrial Corporation Press Brake model AB500-14 when two employees were within reach of the point of operation with the first employee was using the foot pedal.
