29 CFR 1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation.  a) At the constriction site located at 407 67th Street West, Bradenton, FL 34201, the swing of the excavator was not barricaded off to protect the employees from getting struck by the bucket. This resulted in an employees shoulder being struck by the excavator bucket, falling to the ground and having a concussion, and being hospitalized on 12/05/2015.
