29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions: a) 2049 Brown Ave., Bensalem, PA - On or about December 10, 2015, employees were performing residential roofing work , unloading shingles which were lifted to the roof area using a truck mounted crane. Employees were not wearing any head protection while being exposed to overhead hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) 2049 Brown Ave., Bensalem, PA - On or about December 10, 2015, employees were performing residential roofing work , no less than 18 feet above grade, and were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
