29 CFR 1910.178(l)(1)(ii):  The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck.    On or about December 21, 2015, and times prior thereto, the employer did not ensure each employee operating either a forklift or pallet jack had completed forklift training.  This exposes the employees to struck-by and/or caught-between hazards.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.     An in-patient hospitalization of one (1) employee, occurring on or about June 10, 2015, was not reported to the local OSHA area office within the 24-hour reporting requirement.
