29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 2339 East Carson Street, Pittsburgh, PA: On or about January 7, 2016, during roof repair activities, the employer did not ensure that the ladder used to gain access to/egress from the roof, was extended at least 3 feet above the roof line, exposing employees to falls of up to at least approximately 12 feet.  Hooper Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 315560391, citation number 1, item number 3, and was affirmed as a final order on AUGUST 2, 2012, with respect to a workplace located at 1012 Washington Boulevard, Pittsburgh, PA.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record that contained the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:   a) Job site, 2339 East Carson Street, Pittsburgh, PA: On or about January 7, 2016, during roof repair activities, the employer did not ensure that documented fall protection training records existed where employees worked at heights of up to approximately 12 feet.
