On or about January 14, 2016, in the mixing department, employees required to work with oleic acid, morpholine and formaldehyde were not provided with suitable facilities for quick drenching or flushing of eyes and body.

29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids shall be drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve. Transferring by means of air pressure on the container or portable tanks shall be prohibited. a. On or about January 14, 2016, in the storage room, employees dispensed isopropanol, a Category 2 flammable, from a 55 gallon drum without the use of an approved self-closing valve. Employees were thereby exposed to fire and smoke hazards.

29 CFR 1910.134(c)(2)(ii): In addition, the employer must establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user. Exception: Employers are not required to include in a written respiratory protection program those employees whose only use of respirators involves the voluntary use of filtering facepieces (dust masks). a. On or about January 14, 2016, in the workplace, employees were provided half mask respirators but were not medically evaluated for the respirator program.

Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets. a. On or about January 14, 2106, in the work area, the temporary employees were not trained in the hazards associated with chemicals such as, but not limited to, isopropanol and oleic acid.
