29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:     (a) Worksite - Freedom Building Boat Storage: On or about 1/14/2016, the employer did not take adequate precautions to prevent the buildup of flammable vapors in the aft engine compartment of a T-29 Runabout Jet Propulsion boat during a routine maintenance procedure, where an ignition source was introduced causing an explosion.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):     (a) Worksite: Freedom Building Boat Storage:  On or about 1/14/2016, the employer did not assess the hazards associated with using CRC Marine QD Electronic Cleaner in confined boat compartments, to determine if hazardous conditions exist requiring the use of personal protective equipment to protect employees.
