29 CFR 1910.26(c)(2)(iv): Ladder(s) were not maintained in a good usable condition at all times.  On or about January 12, 2016, and at times prior thereto, the employer did not maintain a ladder being used by employees in a good usable condition.  Employees were exposed to a fall hazard.

29 CFR 1910.179(j)(3): A complete periodic inspection of crane(s) had not been conducted in the past 12 months:    On or about January 12, 2016, and prior thereto, the employer had not completed a complete periodic inspection of the cranes in the past 12 months, exposing employees to struck-by hazards.  There were three instances:  1) In the work shop, on the southwest corner of the building there was a 2 ton Conco Crane (Dayton). 2) Out in the yard there was a Petty Bone 32 ton crane  3) Out in the yard there was a 15 ton Grove crane

Damaged or defective sling(s) were not immediately removed from service:  On or about January 12. 2016, and at times prior thereto, the employer was in use of nylon strap slings that were damaged, exposing employees to struck-by hazards.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire.   On or about January 12, 2016, and at times prior thereto, the employer was storing the oxygen cylinders in the same location as the acetylene cylinders, exposing employees to struck-by and fire hazards.
