29 CFR 1910.147(c)(2)(ii): An energy isolating device that was capable of being locked out, did not utilize a lockout device, which rendered the machine or equipment inoperative.  On or about 12/22/205, In the print shop an employee engaged in maintenance activities at the perforator was exposed to amputation hazards. The employee was exposed in that energy control procedures were not utilized prior to changing blades.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  An employee engaged in production activities at the Bag machine #3 was exposed to stuck-by hazards while picking up plastic bags. The employee was exposed in that a guard was not in place to keep the employee from being contacted by the rotating bars at the front of the Bag Machine.

29 CFR 1910.37(b)(4): When the direction of travel to the exit or exit discharge was not immediately apparent, signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge:  In the Print shop, no directional exit signs were apparent to indicate the nearest exit discharge.
