29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  On or about January 20, 2016, the portable fire extinguisher in the Truck Dump Conveyor Tail Pulley pit was not securely mounted or identified as to its location, exposing employees to the hazard of fire and being struck by a falling fire extinguisher.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by ingoing nip point:  On or about January 20, 2016, the tail pulley on the incline conveyor, located in the middle tunnel, was not adequately guarded to protect employees from the hazard created by an ingoing nip point.  Employees were exposed to caught-in hazards.

29 CFR 1910.1200(h)(2)(iii): The location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and safety data sheets required by this section.  On or about January 20, 2016, employees, such as laborers, working around chemicals such as, but not limited to, Polyheed 1020 (Acetic Acid, Ethanol) were not informed of the location of the safety data sheets.  Employees were exposed to the hazards of chemical exposure.
