29 CFR 1926.501(a)(2): The employer shall determine if the walking/working surfaces on which its employees are to work have the strength and structural integrity to support employees safely. Employees shall be allowed to work on those surfaces only when the surfaces have the requisite strength and structural integrity. An employee was installing a wireless internet radio to a tower antenna mount that was not determined by the employer to have the strength and structural integrity to support the employee safely and failed during the installation of the wireless internet radio. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.501(b)(15): "Walking/working surfaces not otherwise addressed." Except as provided in 1926.500(a)(2) or in 1926.501 (b)(1) through (b)(14), each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels shall be protected from falling by a guardrail system, safety net system, or personal fall arrest system. Located on the Westside of a residential structure, an employee was exposed to a fall hazard while working from a tower that was attached to the residential structure and not utilizing any fall protection system. Potential fall height measured between 22'-24 feet above the ground below. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. An employee was standing on the brace section of a tower that was attached to a residential structure without utilizing a fall protection system had not been trained by the employer to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. Employee was exposed to a potential fall height between 22'-24' above the ground below.
