29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    A) West side of house, on or about January 27, 2016, an employee was working from a welded frame scaffold that was not fully planked, exposing the employee to a fall hazard of approximately 15 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:   A)  West side of house, on or about January 27, 2016, an employee was working from a welded frame scaffold that did not have guardrails on the open sides, exposing the employee to a fall hazard of approximately 15 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     A) East side of house, on or about January 27, 2016, employees were performing trim work on windows from an open sided deck/roof of a porch without the use of fall protection, exposing the employees to a fall hazard of about 17 feet.
