29 CFR 1910.135(a)(2): The employer did not ensure that a protective helmet designed to reduce electrical shock hazard was worn by each affected employee when near exposed electrical conductors which could contact the head:  a. At the site, employees were exposed to an electrical hazard while troubleshooting the functioning of a sign, in that, a protective helmet such as a hard hat was not worn, on or about 01/29/2016.
