On or about January 29, 2016, the employer did not ensure that fall protection equipment was used when employees were exposed up to 21 feet to unprotected sides or the edge.  c) Location - 7 Sullivan St. Burlington, MA - Second and third floor stairway.  On or about January 29, 2016, the employer did not ensure that fall protection equipment was used when employees were exposed to unprotected sides or the edge of a stairwell.
