29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.  a) On or about February 2, 2016 the employer had an accumulation of water, trash, electrical cords and other debris on the floor of the Maintenance, Cylinder testing areas exposing employees to slip, trip and fall hazards.

29 CFR 1910.24(h): "Railings and handrails." Standard railings shall be provided on the open sides of all exposed stairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23.    a) On or about February 2, 2016 metal stairway used to gain access to the Steamer was not equipped with a mid rail on one side and standard railing on the open side.
