On 2/2/16, at the McDonough facility, employees were exposed to finger and hand injuries, including amputations on various machines throughout production. On 2/2/16, at the McDonough facility, the Grinding Wheel machine was powered by an electrical extension cord in lieu of permanent wiring for the machine.
