Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees performing maintenance activities: On or about January 30, 2016, and at times prior thereto, during maintenance procedures at 24134 County Road, East/West 181, Chattanooga, OK 73528, the employer did not ensure work platforms on a Lummus Gin Dor-Les Press machine, were guarded by a railing system along all open sides and ends exposing employees to fall hazards of greater than 4 feet.

29 CFR 1928.57(a)(6)(iii): Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment. On or about January 30, 2016, and at times prior thereto, the employer did not ensure that employees servicing the Lummus Gin Dor-Les press disconnect power and wait for the machine movement to stop before servicing the equipment. Employees were exposed to struck-by and caught-in-between hazards.
