29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: The employer is failing to ensure that all employees that are expected to clear jams are adequately trained. Employees are performing activities as affected employees which are authorized activities as per the definition in this subpart. Employees are exposed to flash/fire and amputation hazards, while clearing jammed primers in the machine identified as #1099. This most recently occurred on February 3, 2016 in the facility located at 3625 W. Old Potash Hwy., Grand Island, NE.
