29 CFR 1910.333(c)(10): An interlock system was not returned to its operable condition when the work was completed: (a) Facility- 101 Port Ambridge Drive, Ambridge, PA: On or about February 24, 2016, the interlock on the Ficep machine was not operable at the time of the inspection, exposing employees to the moving parts of the robot and the electric arc when the door was open.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) Facility- 101 Port Ambridge Drive, Ambridge, PA: On or about February 24, 2016, employees were using the Baldor Bench Grinder to sharpen and clean tools and parts. The distance between the abrasive wheel and the adjustable tongue guard measured approximately 1 1/2" apart.
