OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-between hazards.      (a) On or about February 18, 2016, at 6400 West Burleigh St., Milwaukee, WI 53221, an employee was struck-by /caught between a Case 580D back-hoe and a burial vault, when the back-hoe moved forward. This exposed employees to crushing hazards.
