29 CFR 1926.20(b)(2): The employer did not initiate and maintain a program of frequent and regular inspections made by competent persons at the jobsite:        Location: 327 E 64th Street, New York, NY      On or about 2/8/2016      a)  The employer did not inspect the work area to ensure the scissor lift was able to be positioned close enough for the intended work.  Floor holes and equipment pads prevented the lift from getting close enough for employees to safely reach the work. An employees was injured when he leaned out over the railings to do carpentry work.

An employee was leaning out over the railings to do carpentry work and fell approximately 14 feet.
