29 CFR 1910.147(c)(7)(i)(A): Authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  On or about February 22, 2016 and at times prior thereto, at the worksite located at or near 3300 North Hutchinson Street, Pine Bluff, Arkansas, an employee was not trained as an authorized employee in lockout/tagout procedures of the mechanical power press before being assigned to operate the press while the usual operator was sent to lunch. This condition directly led to the hospitalization of an employee.

29 CFR 1910.217(f)(2): Operator was not trained and instructed in the safe method of work before starting work on mechanical power press.  On or about February 22, 2016 and at times prior thereto, at the workplace located at or near 3300 North Hutchinson Street, Pine Bluff, Arkansas, an employee was not trained in the safe method of work before being assigned to operate the press while the usual operator was sent to lunch. This condition exposed the employee to pinch point hazards. This condition directly led to the hospitalization of an employee.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D).  On or about February 22, 2016 and at times prior thereto, at the workplace located at or near 3300 North Hutchinson Street, Pine Bluff, Arkansas, the written lockout procedures for the embossing press did not include gravity in the type and magnitude of energy section. This condition exposed employees to crushing hazards.
