OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to struck-by hazards where unsecured interior and exterior doors leaning against work tables were not secured and/or prevented from falling and tipping over onto employees. Location: 540 West Johnson Avenue, Cheshire, CT 06410 Consequently, On or about 02/29/2016 an unsecured 65 inch wide, 82 inch tall door weighing approximately 200 pounds fell leading against a work table fell/tipped over striking an employee on the back of the head, causing the employee to fall to the floor and sustaining a concussion and hospitalization.
