29 CFR 1926.252(a): Whenever materials are dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, was not used:    a. On February 25, 2016, on the 6:12 pitch roof, employees are throwing tile from the roof to the ground 22 feet below, exposing employees to the hazard of being struck by the tile.

29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a. On February 25, 2016, on the 6:12 pitch roof, employees performing roofing work are not protected from a 22 feet fall hazard by the use of a fall protections system.
