29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: Laundry Room: A door used for exit purposes was unhinged from its jamb at the top. At the time of inspection, the door was locked while employees were working in the area. The door also required a special lifting technique known only to the maintenance personnel to open it.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: 01. Janitorial Closet: A Frank Adam electrical panel board had the entire left side not effectively closed. Four screws which were supposed to hold the metal cover in place at the four corners were missing creating the space on the left side and exposing employees to electrical hazards. 02. Basement, Maintenance Manager's office area: A knockout was missing from an energized electric box exposing employees to electrical hazards.
