(b) Plate Department:  Employees were exposed to amputation hazards from moving parts as the Pendinghaus FPB Plate Punching and Air Plasma Cutting System did not have the expanded metal guard fastened on the north side of the machine.  On or about February 27, 2016 an employee received a partial finger amputation when operating the Pendinghaus FPB Plate Punching and Air Plasma Cutting System on the exit side of the machine without the light curtains in operation.
