29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes. (a) Plate Department: The employer failed to require employee(s) operating the Peddinghaus FPB Plate Punching and Air Plasma Cutting System machine to use appropriate hand protection when employees' hands were exposed to severe cuts from sharp metal, and burns from hot metal.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Plate Department: Employees were exposed to amputation hazards from moving parts, and moving sheet metal when operating the Pendinghaus FPB Plate Punching and Air Plasma Cutting System as the light curtains were not being used for smaller parts on the exit side of the machine. (b) Plate Department: Employees were exposed to amputation hazards from moving parts as the Pendinghaus FPB Plate Punching and Air Plasma Cutting System did not have the expanded metal guard fastened on the north side of the machine. On or about February 27, 2016 an employee received a partial finger amputation when operating the Pendinghaus FPB Plate Punching and Air Plasma Cutting System on the exit side of the machine without the light curtains in operation.
