29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. On or about March 11, 2016, two employees performing roofing operations were not protected from a 27-foot fall hazard by use of a guardrail system, safety net system personal fall arrest system or any alternative fall protection.    b. On or about March 15, 2016, an employee working on the southeast side of a 5/12 pitch roof of a new commercial roof was not protected from a more than 25 feet fall hazard by the use of a personal fall protection system.
