29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder).:  a.  On or about March 14, 2016, at the right side of the house, the fiberglass extension ladder used for roof access was set up at a  57 degree angle with the horizontal distance set at 71 inches for a 131 inches of working length on the ladder, exposing employees to the hazard of a falling ladder.

29 CFR 1926.501(b)(13):   Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.   Employees installing framing and sheathing at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system:  The employer fails to assure effective protection against falls as required by the standard, as demonstrated on March 14, 2016, when employees installing OSB board roof sheathing on the 6:12 pitch roof of a house at 548 Colorado Springs Way in Saint Augustine Florida were exposed to 9 to 18 feet fall hazards above lower levels in that they were not protected by a fall protection system.  Chris Sawdo Construction, LLC was previously cited for a Willful violation of this occupational safety and health standard, which was contained in OSHA inspection number 978000, citation number 2, item number 1 and was affirmed as a final order on October 22, 2014, with respect to a workplace located at 153 South Twin Maple Road in Saint Augustine Florida.  Chris Sawdo Construction, LLC was previously cited for a Repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 879783 citation number 2, item number 1 and was affirmed as a final order on March 5, 2013, with respect to a workplace located at 4668Karsten Creek Drive Jacksonville Florida.  Chris Sawdo Construction, LLC was previously cited for a Willful violation of this occupational safety and health standard, which was contained in OSHA inspection number 315479832, citation number 1, item number 1 and was affirmed as a final order on January 23, 2012, with respect to a workplace located at 11096 Lothmore Road Jacksonville Florida.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.   On or about March 14, 2016, on the right side of the house, the fiberglass extension ladder used for roof access extended only 26 inches above the edge of the roof eave, exposing employees to a 9 feet fall hazard.  Chris Sawdo Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 978000, citation number 1, item number 2 and was affirmed as a final order on October 22, 2014, with respect to a workplace located at 153 South Twin Maple Rd in Saint Augustine, Florida.
