29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a.  On or about March 16, 2016, on the northeast side of a 6/12 pitch roof of a residential house, an employee working on the roof was not protected from a 19 feet fall hazard by the use of a personal fall protection system.  b.  On or about March 16, 2016, in the center of a 6/12 pitch roof of a residential house, two employees working on the roof was not protected from a 19 feet fall hazard by the use of a personal fall protection system.  c.  On or about March 16, 2016, on the northwest side of a 6/12 pitch roof of a residential house, an employee working on the roof was not protected from a 19 feet fall hazard by the use of a personal fall protection system.
