29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) Jobsite, Building 7, Oakhurst Extension, Johnstown, PA:  On or about March 21, 2016, an employee was installing siding from a ladder jack scaffold and was not utilizing fall protection.  The fall hazard to the employee was approximately 12 feet to the ground.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:  a) Jobsite, Building 7, Oakhurst Extension, Johnstown, PA:  On or about March 21, 2016, an employee was exposed to fall hazards while installing siding and the employer had not trained him to recognize the hazards or how to minimize them.
