29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: a. At the fourth floor, employees were exposed to a 30 feet fall hazard, in that, 13 windows which were 10 inches from the floor were not provided with a midrail, on or about 03/24/2016.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan: a. At the site, employees were permitted to use a pan style stairway with three flights of stairs which was to be filled with "lightcrete" and was not fitted with wood so that the tripping hazards at the landings and at the top of the stairway were eliminated, on or about 03/24/2016.
