The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Willow Valley Communities Club House Bowling Machines - Employees walked and worked performing service to the 6 Brunswick A2 Pin Setters at the rear of the alley and were exposed injuries from the rotating parts and flying bowling pins from the unguarded entrance at the back of each of the 6 Pin Setters, on or about March 29, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement
