29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:     (a) Willow Valley Communities Club House - The employer's Lockout/Tagout Program did not include machine specific procedures that clearly explain the process to be followed for the bowling alley pin setting machine, on or about March 29, 2016.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  Willow Valley Communities Club House Bowling Machines -  Employees walked and worked performing service to the 6 Brunswick A2 Pin Setters at the rear of the alley and were exposed injuries from the rotating parts and flying bowling pins from the unguarded entrance at the back of each of the 6 Pin Setters, on or about March 29, 2016.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):     (a)  Willow Valley Communities Club House Bowling Machines -  Employees performing service to the 6 Brunswick A2 pin setters at the rear of the alley and were exposed to injuries from unguarded pulleys driving the ball return mechanism and the pin set mechanism at the back of each of the 6 pin setters, on or about March 29, 2016.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven -7 feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt:    (a) Willow Valley Communities Club House Bowling Machines -  Employees performing service to the 6 Brunswick A2 pin setters at the rear of the alley and were exposed to injuries from unguarded horizontal belts at the back of each of the 6 pin setters, on or about March 29, 2016.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
