29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) At Gulf Shores Surf and Racquet Club Gulf Shores, Alabama: On or about April 5, 2016 and at times prior; the employer allowed his employee to work from an elevated aerial lift basket without a harness exposing him to a fall hazard of 11 feet to ground below.
