29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: as part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.    (a) Located on the existing platform area of a grain elevator's leg, worker(s) were exposing to a potential fall hazards while demolishing a section of the elevator's leg in that the PFAS was not independent of the platform supporting the worker.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer shall have in writing evidence that such a survey has been performed.      (a) Employees were engaged in commercial construction activities demolishing and installing grain elevator storage equipment. Workers were exposed to potential fall and struck-by hazards in that there was not a proper engineering survey by a competent person.
