29 CFR 1928.57(a)(6)(iii): Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment. a) On or about April 21, 2016 and at times prior thereto, employees were not instructed to turn off power to equipment while servicing. An employee was greasing equipment adjacent to chain and sprockets with the engine running and PTO engaged. NOTE: Abatement certification AND supporting documentation are required for this item.

29 CFR 1928.57(a)(11): Where a moving machinery part presented a hazard during servicing or maintenance, the engine was not stopped, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed: a) On or about April 21, 2016 and at times prior thereto, employees were exposed to amputation hazards while lubricating bearings and chains on manure spreaders, where employees did not turn off power to equipment while servicing. NOTE: Abatement certification AND supporting documentation are required for this item.
