29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):    a)  Moulding Building, 248 Calvary Church Road, Melcroft, PA 15462:  On or about April 22, 2016, the employer did not establish energy control procedures or provide employee training for the serving and maintenance of an Eckstrom Carlson Series E brand straight line rip saw.  Employees were exposed to cut-by hazards while using their hands to clean debris from the blade compartment of the straight line rip saw while the saw was not locked and tagged out.    b)  Moulding Building, 248 Calvary Church Road, Melcroft, PA 15462:  On or about April 22, 2016, the employer did not develop energy control procedures or provide employee training for the serving and maintenance of a dust collection system used to exhaust sawdust from the Eckstrom Carlson Series E brand straight line rip saw.  Employees were exposed to cut-by and struck-by hazards while using their hands to remove debris from inside the dust collection system within a few feet of the exhaust fan.  The exhaust fan was not locked and tagged out while debris was removed from the duct work.
