29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system :  a- Residential Roof:  Employees were exposed to fall hazards of approximately 18ft to the ground below as a means of fall protection was not utilized. Condition noted on or about 04/21/2016.

29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive siderails:  a- Residential Roof: Employees were exposed to electrocution hazards in that an aluminum extension ladder used to access a roof was within an energized secondary power source which was not de-energized. Condition noted on or about 04/21/2016.
