29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems.    (a) Location - 18 Lexington Avenue, Lexington, Massachusetts:  On or about May 2, 2016, employees performing residential construction at a heights of up to 21 feet 3 inches above the ground were not protected from falling to the ground below.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge.    (a) Location - 18 Lexington Avenue, Lexington, Massachusetts:  On or about May 2, 2016, employees utilizing stairways at a residential construction project were not protected from falling by a stair rail system.
