Stair width was measured at 16 and 1/4 inches.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   a)Logan Machine Works, Inc.:  On or about May 5th, 2016, and prior to, employees were exposed to inhalation hazards from welding fume and metal dusts generated from grinding.  The employee(s) had not been provided training on dust mask respirators to include Appendix D and were provided and wear dust mask respirators.
