29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  East side of building:  On or about April 29, 2016, an employee was exposed to a fall hazard greater than ten feet to the dirt ground below while placing roofing material on the window awning from a temporary elevated work platform on a Carelift Zoom Boom Forklift.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):   Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178.   29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l) AND AS REFERENCED BY THE NOTE CONTAINED IN 1926.602(d):  East side of building:  On or about April 29, 2016, employees were exposed to fall and struck-by hazards, while operating a Carelift Zoom Boom Forklift, Model 8040, with a temporary elevated platform to raise roofing material and employees.
