29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) 2 16th Street, Wheeling; First floor: On or about January 15, 2016, employees cut out a 8'x20' foot floor hole for a new stairway installation. Employees were exposed to a 16 foot fall hazard from the unprotected floor hole. (b) 2 16th Street, Wheeling; Second Floor: On or about January 15, 2016, employees cut out a 8'x20' foot floor hole for a new stairway installation. Employees were exposed to a 11 to 27 foot fall hazard from the unprotected floor hole. (c) 2 16th Street, Wheeling; Third Floor: On or about January 15, 2016, employees cut out a 8'x20' foot floor hole for a new stairway installation. Employees were exposed to a 11 to 38 foot fall hazard from the unprotected floor hole.
