29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) PADC2, Sortation Department, Dunnage Area, Stations 305/306 - on or about May 21, 2016, the employer did not train each employee unjamming paper in the Sealed Air PackTiger paper cushioning systems in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: a) PADC2, Sortation Department, Dunnage Area, Stations 305/306 - on or about May 21, 2016, employees unjammed paper in the Sealed Air PackTiger paper cushioning systems, reaching into the chute opening and pulling paper away from the cutting blade; the employer failed to ensure the control of energy by use of unplugging the cord and plug, exposing employees to caught-in hazards.
